Request for absences
Use the button below to request permission to attend appointments during the school day, to ask for term time leave, or leave for religious observance.

Please note that days taken which bring the total number of days for religious observance above two will be unauthorised.
Permission to administer medication
Use this link to request that your child has medication administered. This should be prescription only and should be taken at home if possible. The medication must be presented in a labelled container showing the child’s name.

Free School Meals and Pupil Premium
For families of existing pupils who wish to apply for Free School Meals, due-to a change in circumstances, you will need our form for existing pupils (click on the button below).
If submitting an in-year transfer from another school, you can provide a letter from your child’s current school which states that your child is eligible for Pupil Premium, and is eligible for benefits related Free School Meals, as supporting evidence. This should have the start date that your child became eligible and an end date if applicable.
